Know what the medical record shows before anyone else does.
Healthcare cases are won and lost in the medical record. Lexcura Summit reads that record the way a clinician does, so attorneys can see the strength of a case before they invest in it, and healthcare organizations can find their documentation risk before an attorney or a surveyor finds it for them.
Every review follows The Lexcura Clinical Intelligence Model™ and its seven pillars. Clinical findings are made or verified by clinicians holding an active registered nurse license or a higher clinical license. Trained analysts, including paralegals, organize records, build timelines and check citations under clinical supervision. Every report is fully checked before it leaves our hands.
Paid in full before work begins. Records accepted only through our secure portal under a signed Business Associate Agreement. A conflict check before every engagement.

For Attorneys and Law Firms
Know what you have before you spend on it
A healthcare case can look strong on intake and fall apart in the chart. We read the records first, so you know what you have before you spend money, time and credibility on it. You'll see the evidence plainly, including the parts that hurt your case, so nothing surprises you in a deposition or at trial.
Acute care and every case type we review
Every attorney service below applies to hospital and acute care matters as fully as to long-term care, for plaintiff and defense counsel alike.
Medical Malpractice
Personal Injury
Wrongful Death
Negligence
Long-Term Care and Nursing Home
Mass Tort
Emergency Department
Triage, missed or delayed diagnosis, and discharge decisions.
Hospital and Inpatient Care

Diagnostic Delay and Imaging
When findings were available, who saw them, and what was done.
Attorney Case Brief
An early, clinician-level read of one matter.
Before you take a case on, or before you spend more on one you already have, the Attorney Case Brief tells you what the records actually say. We pull out the key clinical facts, identify the standard-of-care questions that matter, look at causation, and give you an initial view of the case's strengths and weak spots. It's built for the moment when you need to decide whether to go forward.
What you receive
Key clinical facts drawn from the record
The main standard-of-care questions
An initial causation assessment
Strengths, weak spots and open questions
A list of missing records, if any
Turnaround: 30 business days from receipt of complete records. Rush: 20 business days.
Three Case Briefs for your firm's first engagement.
The best way to see what clinical intelligence does for a practice is to try it on real cases. This introductory package gives your firm three Attorney Case Briefs at a reduced rate, so you can compare our findings against your own instincts on three different matters.
What you receive
Three Attorney Case Briefs, each covering up to 500 pages
An optional call per case to walk through the findings
Available once per firm. All three cases are submitted within 90 days of purchase.
Everything you need from the records to build a strong demand.
When a case is going forward, the demand has to stand on the medical record. The Demand-Ready Package gives you three reports that work together: a medical chronology that lays out what happened and when, a narrative summary written to support your demand, and a record-gap list so you know exactly what's missing before the other side points it out.
What you receive
Medical chronology
Narrative summary with demand support
Record-gap list
Additional pages are counted at the start and invoiced before work begins.
The complete seven-pillar review for cases headed to litigation.
For complex or high-value matters, the Full Clinical Intelligence Analysis takes the entire record through all seven pillars of The Lexcura Clinical Intelligence Model™. We check the integrity of the record, establish the patient's baseline, review imaging and other fixed studies, reconstruct the timeline, measure the care against the standard of care, apply the regulatory and compliance requirements that governed it, and map causation from each decision to the outcome. You see who knew what, and when.
What you receive
A full written analysis with timeline reconstruction
Clear, accurate summaries of medical records, prepared by licensed clinicians.
Thousands of pages of records aren't useful until someone who understands them tells you what's in them. Our medical summaries turn a stack of records into a clear, organized account of the treatment, written by clinicians rather than generated by software, and fully checked before delivery.
Find the gaps first, while there's time to fix them
When something goes wrong, the chart is the first thing anyone reads, and it's read by people looking for gaps. We help hospitals and health systems, senior living communities, home health and hospice agencies, LTACH and rehabilitation hospitals, and behavioral health providers find those gaps first. Risk and compliance teams across all of them use our reviews as an early-warning system, and every facility report flags standard-of-care and regulatory gaps clearly, so your team knows exactly where to start.

Who we work with
Hospitals, risk and compliance teams, and the attorneys who specialize in these cases — every service on this page is built for them.
Hospitals and health systems
Acute care carries the same documentation risk as any facility, with more departments, more hands on the record and more places for the story to break. Every attorney and facility service applies to hospital care in full.
Risk and compliance teams
This is where we do our deepest work. Risk managers and compliance officers use our chart audits, policy-versus-practice checks, survey readiness reviews and the quarterly retainer to find and fix gaps long before a surveyor, an attorney or a plaintiff's expert ever reads the chart.
Attorneys who specialize
Plaintiff and defense attorneys who concentrate on medical malpractice, personal injury, wrongful death, negligence and long-term care use our reviews to know what the records actually say before the case is priced, settled or tried.
Leadership Briefing
What an attorney or surveyor sees in your records, in 45 minutes.
A short, practical session for your leadership team on how outside reviewers read your charts in the first hour, where documentation most often breaks down, and what you can fix now. No patient records are shared.
Format: 45 minutes on Zoom (30 minutes of content, 15 minutes of questions). In person in North Central Florida.
Training your staff on documentation that holds up.
Your nurses and aides write the record every shift. This in-service shows them what good documentation looks like when a fall, wound or change in condition is later reviewed, using real-world examples and clear, practical habits they can use right away.
A clinical review of one serious event, before anyone else reviews it.
After a fall with injury, a pressure injury, a restraint, an elopement, an assault or a self-harm event, you need to know what your records show. We review the event and the documentation around it and give you a written risk memo covering the timeline, your exposure, any standard-of-care and regulatory gaps, and corrective steps.
Turnaround: 30 business days. Rush: 20 business days for an additional $1,500.
Know your exposure before you release records to an attorney.
When an attorney requests a chart, that request usually means a claim is being considered. Before the records go out, we review them and give you an exposure summary, so your leadership and counsel know what the attorney is about to see.
We review ten records from one facility ahead of your survey, rank each gap by citation risk, and give you a fix-first plan so your team spends its time where it matters most.
Choose the level of review that fits your organization.
Chart Snapshot
$2,950
Five charts from one facility, reviewed against the standard of care and regulatory requirements, with a one-page findings sheet. A low-risk way to see what we find. The fee counts toward the Lexcura Pilot.
Policy vs. Practice Check
$5,950
Do your charts follow your own policies? We test four of your written policies, such as falls, wounds and skin care, and change in condition, against 12 charts where each policy applied, and give you a side-by-side comparison with gaps ranked by risk.
The Lexcura Pilot
$15,000
A full look across your organization: 30 charts across three facilities, a written report and a findings briefing for your leadership team. A Single-Event Review can be added for $4,500.
Ongoing chart audits that keep your documentation survey-ready and defensible.
For multi-site organizations that want continuous oversight rather than a one-time review. We audit charts across your communities every quarter, track findings over time, and report to leadership, so problems are caught and corrected long before a survey or a lawsuit.
Custom proposal, priced per community per quarter.
$350 per additional 100 pages, confirmed with you before review continues.
Rush delivery
20 business days instead of 30. For attorney services, 50% of the service fee. For facility services, $1,500.
Follow-up call
An optional 20-minute call to talk through your report, included with every report.

Every report is fully checked before it's sent directly to you. Standard turnaround is 30 business days.
Lexcura Summit provides clinical analysis, not legal advice. We run a conflict check before every engagement and do not take matters adverse to a current client. All services are paid in full before work begins.
Training, Certification and Licensing
Clinical intelligence can be taught
Our training programs give nurses, paralegals, analysts and legal teams a structured way to read medical records like a clinician and turn them into evidence, using the same model we use on every case.
A full training program in The Lexcura Clinical Intelligence Model™ and its seven pillars. Participants learn to assess record integrity, establish a baseline, reconstruct a timeline, measure care against the standard of care, apply the regulatory overlay, and map causation, and they earn the LCCIA credential.
For certified professionals and firms that want to use The Lexcura Clinical Intelligence Model™ commercially in their own work. Licensing is especially suited to larger firms that certify their staff and want to bring the model in-house.
Certification must be completed first. If your certification was completed more than 24 months ago, the Certification Refresher ($995) is required before licensing.
In about two hours, learn to read medical records the way a clinician does and turn what's in them into evidence. Narrated by Michelle Carroll and available to watch at your own pace.
On-demand CLE courses on medical records, clinical evidence and healthcare litigation, approved for attorneys in Florida, with course numbers and credit hours listed for every course.
We believe attorneys and healthcare leaders make better decisions when they understand the clinical record. These resources are here to help, whether or not you ever engage us.
