What do you need the review to help you decide?
An initial enquiry is most useful when it identifies the care setting, the disputed event and the next decision. Keep it high-level: you do not need to send a patient's record or detailed medical history to begin a discussion.
The practical constraints
Include the relevant care setting, approximate record volume and any known deadline. Mention incomplete records or a specific unresolved discrepancy without copying sensitive chart information.
